Something seems off.
Google search the images and one is a Toronto Kijiji ad.
One picture is for a different massage place/name but very similar (almost identical) web page
No way to contact other than email form on site.
No address or phone number
Think I will look elsewhere.
 
Perhaps simply "I require a deposit for first time customers". If you send it you are out $50.00.

Or "please complete the attached health questionnaire", which when you open it pffffft they have control of your computer.
