Approaching client's doctor

For the first time since coming out of massage school, I am working with a client with an extensive and complex medical history, including congenital heart failure.

I have not yet gone through the process of getting client approval, contacting the md's involved, and eliciting approval to proceed with massage, and have since misplaced my notes pertaining to this.

Any suggestions for proper proceedure or web links/ books that will put me in the right direction would be of great help.

Hi maestra. I've been doing more lurking than posting, but I'm still around.
How've you been?

I checked Sohnen-Moe; she has HIPPA and some "request for access to files" type forms.

In Path class, we went through a whole proceedure of CYA, with pretty concise language for client approval as well as md approval. I assumed it was more or less standardized, but in trying to locate resources, I'm beginning to suspect that it was just our school's approach. And maybe, as pueppi suggests, it doesn't need to be quite so formal.

I just don't want to start working on this guy with the list of issues he has without some kind of medical consent.

Also, like pueppi suggests, I would love to begin developing relationships with doctors in the area, and I think this is a perfect way to do it.
