Client Feedback Forms

I would like to have a mechanism through which my clients can give feedback on their experience. Cards to mail back don't seem to work (low rate of return and they're giving their viewpoint days or longer since the experience has passed). I thought for sure I remembered at one point someone giving a free website that one could set up an account and a link from their site to make giving feedback easier. While it's not instant, it might be a nice addition for my internet saavy clients. I have searched with no luck. Anyone remember this?

I have also considered putting feedback response cards in the massage room. For those of you who put feedback cards in the office, what has your response and experience been?
