Hi allGreat forum!!I'm not sure if this topic has been covered but I'll ask anyway.  When taking a new clients details, medical history/ background etc do you use a form that you have put together yourself & that is applicable to your clinic or style of body work or do you use a general one that is available from a massage equipment supplier?Hope this makes sense.All feedback greatly appreciatedThanksLindyAustralia

I tweaked a medical intake form I found online to accomodate my needs/requirements out of a form.

If you're an ABMP member, there's a version of a client information form at the back of the business handbook you get as a member.  I've recreated it, adding info, and removing what I don't think is necessary.  What I'm going to add next, though, is a section to print on the back page of the front and back of the body, so they can circle areas of pain, put an X on areas of tightness, etc.

Same here... I used a pre-made form for a start off point.I also use a different intake form for my prenatal clients.My policies are listed individually on a second sheet of paper.  The client reads, checks off each one, and signs.

Lindy, have you just finished a Cert IV in massage or something?  You should've covered intake forms in the course, it's part of the Australian standards required for the course.I got my basic form in class and have been tweaking it ever since as I've added other stytles, need more details.  I check out online versions and always look at any intake forms I fill out to look for good ideas.I don't have any graphics on the form to indicate "where are you feeling pain?" or similar, but it's the one thing I always think I should add when I tweak.

I tweeked the one I used in schoolnow that you mention it I think its time for an update
