Client Information for Marketing

I've been growing my business for the past year and I've discovered that I don't have all the information I want on my client intake form. I'm designing a Client Information card using a vistaprint postcard. My intention is to ask my clients to complete this information card on their next visit to my office.

Here's the info I'm gathering:
Client Last Name
Client First Name
Birth Month and Date (for future "It's your birthday, here's a discount!" promotions)
Email address
Mailing Address
Appointment Reminder Preference: Email, Text or None
Notes

I'm closing with this statement: Your personal information is confidential and will not be shared.

My question for you is this: What information do you wish you had on your client base? Am I missing something?

100 postcards will run me twenty dollars. They'll match my business card design and meet my aesthetic needs.

I know I have these two questions (off memory - won't be back in the office until Wed.):

• Who Referred You? - always very helpful to know
• In Case of Emergency (Name & Number) - I've had to use this once for an elderly client who had a TIA while on my table and since I didn't want her driving home, I called the ambulance and the emergency contact
