I really dont have a problem with no shows but it does occur occasionally and I have decided to add another section to my  client intake formate  made contact with new client________Date & time first appt___________Notified client of cancellation policy_____Gift Certificate #_______Date follow-up call____________The rest of the intake is medical info etc and on the back of client intake is 4 treatment records.Please share if any other ideas

I am a little confused.  This information is listed on the intake for your reference, right?Do you have your cancellation/no show policy posted or printed anywhere?Is it on your intake forms or are you only verbally telling them?

Yes, the information would be for my reference.  I have my cancellation policy posted in full view on an office wall, in my "thank you letter" and on the client intake form.  The information regarding policy, price, directions is passed on to the client during scheduling.

On the day of the  appt, the client must read and sign the client intake form before the massage.

The one thing that I do on my intake form is have the client mark a check after each of my policies.I have a second sheet with my policies listed.  They check each one and then sign.  I like this because it seems to make them read the policies instead of skim over them and then sign.  I noticed many would skim over them when the policies were all listed in a paragraph at the end of the intake.

I really like the idea of a second sheet/page with office policies and sign off.  I do believe that I will incorporate it into my practice.  Thanks
