Hi,

Its lazy i know but can anyone email me a decent, basic questionnaire for reflexology that i could use?

Ive been in business less than a year - doing colonic hydrotherapy - and now wish to add reflexology to my treatments.

i trained 10 years ago!

so i need some help.

I will be scanning these pages to sus out whats new in the foot world.

There's a thread on this in the student and training forum, which contains lots of skeleton samples, which you could use as a framework.

There are sample forms on the ITEC site, which you could use and adapt. They have one for each therapy.

I buy my consultation forms from www.fht.org.uk - they are double sided and printed in a nice ink too with a foot diagram on the reverse.
They go into quite a lot of detail also about a pregnant client too.

Have a look at their catalogue on their website - the do some for IHM, Aromatherapy and Massage too.
