When I'm asked to fill out forms at a spa (not a clinic) where I had no intentions of filling out forms, I always note - Seeking Treatment for DSB. If asked further, I verbalize one on one 'Dreaded Sperm Buildup'. I have never been thrown out.

Ha Ha Ha DSB that a good one.
A few years ago I put on a form . I have a severe groin injury which I would like to be worked on.
Well it worked I got a really nice undraped groin and inner thigh massage. Nothing more but it was nice .
