Doctors Consent

Hi all, wonder if you can help with some advice.
My friend is getting ready to take her IHM ITEC exam.
As part of the exam she has to complete template letters for her client's GP, in which the GP can give or refuse consent for the IHM treatment.
Has anyone else had to do this as part of their exams, and if so, what should the content of these letters be? Any help would be much appreciated.

Hello YES I had to design doctors letter for reflexology/aroma/swedish:

Dear Dr Starbuck.

Re: *******, ADDRESS********D.O.B [/b]01/01/44[/b][/b]

The above patient has approached me for a course of Reflexology.
During the consultation process, it has become apparent that your patient maybe contraindicated to the treatment due to high blood pressure.

As a holistic therapist I would like to point out that reflexology is a Safe and Natural treatment, however in some circumstances it may not be appropriate for some patients to receive the above treatment.

I would be grateful if you could sign and return this form in the envelope provided at your early convenience.

Thank you for your assistant in this matter.

Just change reflexology and put indian head and of course you would have to change the contraindications.
