Just come up with something to say like maybe "Ya know, we're lab partners and I don't even know your name. I'm <insert your name here> by the way" and then try to strike up normal conversation.

I've had the same problem! You just have to say, Hi, I'm...." What's your name? Or say something random about what you're doing, unless he's really uptight he should answer. When I had my lab partner, the teacher was crazy, and I would always make fun on him. My lab partner was laughing and we became really good friends.
 
Haha happens to me sometimes where i don't know there name or never really talked much, just say so whats your name,he answers, you tell him yours and just be friendly to him while you work togehter
