Now I knew there were innie and outie belly buttons, but for the life of me I never knew there was also innie and outie ass holes.

My non-medical professional guess would be that she had external hemorrhoids, that's when the veins around the anus become inflamed and swell.
There is such thing as having a rectal prolapse however I believe in humans it's rare (in some animals it's fairly common), and not likely that she was suffering from this.
One of the symptoms of hemorrhoids is that it is itchy, that might explain why she did not find it comfortable but I'm guessing here.

Hemorrhoids take many different shapes, it is entirely possible that this was the cause of her outie - inflamed doesnt necessarily mean there has to be a giant tumor sized growht, it could just be a minor swelling.
