It’s time to renew your Georgia Massage Therapy license. Your license expires October 31, 2014, so visit http://www.sos.ga.gov/plb anytime before then to renew. Click on the License Renewal link to begin the renewal process. The online renewal process takes only a few minutes. After updating both your physical and your mailing addresses, and your email address, and answering the renewal questions, you may pay using your American Express, Mastercard, or Visa using the secure server.
RENEW NOW! Do not wait until the deadline. It may take up to 10 business days to update in the system, so please ignore this email if you have.
Georgia law now requires those applying for renewal of an existing license to submit an Affidavit of Citizenship with their application.* The Affidavit of Citizenship was mailed to you. *You must complete the affidavit of citizenship and have it notarized.* Mail your completed Affidavit of Citizenship to the Board office using the form that was mailed to you.* If you did not receive a form in the mail, please download one from the Board’s website to submit.** Please allow 10 business days for staff to process your secure and verifiable document and renewal application.
Your license renewal will not be completed until you have completed the online renewal, submitted the renewal fee, Affidavit of Citizenship and documentation to compete your Continuing Education documents, if selected for audit.
Please do not submit continuing education documents unless you are selected for audit.

• Click on the License Renewal link to begin the renewal process.

• Update your mailing and physical location addresses, phone number, e-mail address and answer the renewal questions.
• Mail your Affidavit of Citizenship following the instructions that were mailed to you.

• Print your receipt of payment.
• Verify your renewal online.
• Print your license online.

If you choose not to renew online and would like to request a renewal form that will be mailed to your address on file with board, please call 478-207-2440.
Please note that renewing by mail may take up to 4 weeks to process after the completed renewal form is received. A late fee will be imposed if you renew after the expiration date of your license. You cannot practice after your license expiration date.
