Ditch the pads! This is only confusing your puppy and actually encouraging her to do her business indoors. You need to get in tune with her 'needs' - ie get her out after every nap, feed, short period of playtime. She'll need to do this roughly every hour (depending on whether she's over 14 weeks or not) at least to begin with. And if you've timed it wrong (she should actually go immediately she is taken outside) and she goes when you bring her in instead, stay with her in the room by the door and the moment you see her go to squat, say No, calmly and firmly so you don't actually frighten her, and take her outside. And this is where a crate can be useful because ideally you bring her in if she's not done anything after no longer than 10 minutes, and crate her, leaving her for another 10 minutes or so, and repeating until she does do something. Then you give her loads of praise.
Her mistakes are your mistakes. Always remember that. There will be mistakes however, it's all part of having a puppy, but the fewer she has, the faster she'll learn what you are trying to get her to do, and where.
As for overnight, you'd be far better getting up at least once during the night and taking her outside rather than leaving her to pee etc. while locked in the bathroom. That's counter-productive, even if it's the 'lazy' option. Again having a puppy is a lot of work, but if you are going to get anywhere near her being clean in the house, it has to be done. And rather than shutting her in the bathroom overnight, why not bring her, in her crate, into your room with her so you'll know when she stirs during the night, and needs to empty.
