I've done many an event of the years. I have to admit I just do it verbally. Some therapists have a laminated sheet with typical questions like "Do you high blood pressure?" etc. Others do a more comprehensive questionaire, which seens a bit over the top for a taster session. If you want to do it on paper, keep it short and sweet, more more than A5. After all they come for a relaxing treatment, so best not to stress them out with 5 sides of A4, lol The most important thing is that you have make the effort to filter "potential" contraindications.
Typical questions to ask:
Hi BP?
Diabetes?
Foot problems? (including infections)
Asthma? (not necessarily a CI but nice to know just in case)
Epilepsy? (not necessarily a CI but nice to know just in case)
Prehaps pregnancy (not a contraindication in my way of thinking as its a natural event not a medical problem)
My favourite questions are:
Is there anything about your health that I should know about? (Because it puts the responsibility onto the client.)
and
Are you on any medication - conventional or alternative? (As this draws out any health issue, as there must be a reason why they are on meds.)
These two questions give an instant snap shot of your client.
