Triple Hiatus Hernia - doctors note required?

I have an enquiry from a guy whose wife I did an Indian Head Massage on a few weeks ago. He is wanted a reflexology treatment and after in a week or so he wants Indian Head massage. He has a triple hernia and is under 30. I have suggested he gets checked out by a doc again as he is in alot of pain but he says he has been to the docs and they just give him tablets that make him hallucinate. So therefore he came off them himself. He is on another lot of meds but would you get a docs note first to confirm it is safe - I am sure reflexology would be fine and would ease pain but what about Indian Head Massage - I am sure IHM would be ok without a docs note?
I will ask him I think how severe the pain is and ask him what meds he is on and maybe suggest he gets a docs note first ?
I would appreciate your opinions as soon as anyone can help. I will ring him shortly.

What would you suggest to concentrate on whilst doing a reflex treatment. I do carry out a general all over foot treatment as suggested in our training but i like to go back and concentrate on any problem areas at the end.
WOuld you avoid the are where he has a pain - you see this is the trouble with reflexology - if people come to you to relieve pain it is difficult to know when to avoid the area and concentrate on other areas or to manipulate the area albeit very gently? HELP PLEASE!!
