TTC Questionnaire

Hello

Just wondering if anyone uses a particular questionnaire for those clients trying to concieve? (..and if you do, could I possibly ask if you could PM a copy to me pretty pretty please??!!)

I dont think my general one targets the right areas.....any thoughts?

Hi

I do a lot of fertility work and to be honest I use my general consultation form as you are treating the whole body and not just the reproductive system. I add all the info in the reproductive system box. Diet ect gets talked about generally anyway....

Things i ask are

how long they have been actively trying
what is cycle like,regular,heavy/light/patchy/smelly
how often they have intercourse (not going to get pregnant without sex more than once usually!)
Have they any other children, (primary or secondary infertility)
Have they been referred to a specialist
Are they having or had fertility treatment / what type/ good response
Have they had a pregnancy form fertility treatment whether miscarried or not

Thats from the top of the head now.. may think of something else later

Ive devised my own form as I seem to be getting more and more ladies ( & gents!) for this. I have to same or similar questions as yourself, so at least I know Im on the right track!
