I have a client that has an impinged sciatic nerve due to having her tubes tied.  So far I have striped, kneaded, static compressed the piriformis.  She told me the following day that the pain was worse.  We both agreed that it was possibly due to her not being used to regular treatment.  She is coming back tomorrow and I plan to do more stretching and alternating heat and ice.  Has anyone else delt with this type of impingment?

Unless there was a surgical misadventure I cannot fathom how one has to do with the other.

Her legs would have been up for the procedure but it only takes(on average) 15 minutes....if that.

I don't think her legs would be up, but she could've been on the operating table awhile.  This procedure is usually done with a laproscopy........tube to see the tubes and then cut them or tie them.  I've seen some clients (in home care) who have had surgery and have all sorts of complaints.........sore shoulders, sore upper back, and back pain. Have you suggested a chiropractor to try to realign her?  Or accupuncture?  Just a thought.  How long ago did she have the surgery?

nybor7,I am not sure, but my thought would be that under anesthesia, she possibly was moved in a funky position while putting her on or off the surgical table. (?)I have never seen the proceedure performed, however, I have watched other surgeries and seen the patient "flipped" or "moved" in ways that I personally think would not be benefical to the spine.  Especially since with anesthesia the patient can't guard a body part.Just a thought.  Sounds like you've been in on the proceedure, so maybe you can shed some light.  Is it a "general" or a "local" that is usually given?

We take every caution to not put them in funky positions.  

{I am an OR Tech by day, I assist the surgeon in the perioperative care of the patient}

Yes, to the post above, the legs are up.  True, it is done laparoscopically but she is a "lithotomy" position because there is a device inside her vagina attached to her cervix so her uterus can be manipulated(boy, that sounds bad when you type it out).

Positioning IS the only thing I can think of also...I was reading as the procedure may have caused the sciatica.

If anything out of the ordinary happened(even a bruise) it would be noted and an incident review made.

Unique to us as massage therapists IS a referral of shoulder pain in most instances after any laparoscopic surgery.  The insufflation(adding air into the belly) is something the diaphragm "doesn't like" and it refers pain to the shoulder....

I also do know that "things happen".  My mother had a total knee replacement with no surgical misadventure.  Although her new knee is perfect she has excruciating pain along her IT band and no one can attest to what it is.  It was most probably due to the tourniquet, or drain.

I'm just still here sitting scratching my head about the cause.  

For help, for I am a sciatic sufferer.....I have psoas worked on like mentioned above...and use heat heat heat whenever I can.

Yes I have.

General via et Tube.

She probably got local injected into her trocar sites.

just an idea here - my personal hx - was there any emotional trauma around the procedure?  after my premature daughter was born and i had my TL at her birth, a 'series of unfortunate events' happened and i, foolishly, held onto pain much longer than i should have out of my own guilt.she may not be the type of person to do that - but it could be a possibility.  good luck

well, i don't even know where to start, really.i had very difficult pregnancies that both resulted with premature deliveries.  both deliveries were incredibly scary, but the second, of my daughter, was the scariest.i can actually 'touch' the moment that my brain fried.  there i was, on the operating table at the same time being told by my OB that he was completing the tubal while overhearing the pediatrician saying, 'come on, little girl, breathe'.what followed was months of self-inflicted torture because i didn't stop the OB and try to keep her in longer.  the truth was, i was so tired.  she was only 6 wks early and i'd gone into labor at 24 wks - so i'd been having a rough time.  but i beat myself up for not being stronger.she was fine - 10 days in NICU but never needed a vent, but the sights i saw and the separation from her from day one really affected me.  i walked around for six weeks with what literally felt like daggers in each hip.  it wasn't until i started to deal with it that the pain let up, and nearly 3 years later i still feel the phantom pain when she has an asthma attack.i know that for me, it's all emotional and i still have quite a bit of baggage to work out - and i know that i am the one holding on to it.  but the reason i shared this is that i was totally in denial right after her birth and sought any remedy possible to help with the pain.  it's a  wonder i didn't get addicted to any medication.  sometimes in a person's intent to be strong and stoic, we forget that we're also vulnerable at times.  i don't know, this particular client may not have had anthing at all like this, but sometimes when you can't find a tangible reason for something it's good to look at other possibilities.hope i made sense with this, and if anyone that reads this has gone through a similar situation, or worse, i hope i haven't offended you in anyway.  not everyone reacts to this situation like i did.  there are millions of women who are a lot stronger than i was and never had the opportunity to bring their children home.amanda

hope i made sense with this, and if anyone that reads this has gone through a similar situation, or worse, i hope i haven't offended you in anyway.  not everyone reacts to this situation like i did.  there are millions of women who are a lot stronger than i was and never had the opportunity to bring their children home.

Reminds me of an article in one of the massage magazines a few months back regarding Psoas.  It held that there was an emotional component with psoas issues.  Don't have it onhand to referene...I'll try and find it.

Interesting...Did she have her tubes tied after she delivered her baby?  Did she have an epidural?  And what number pregnancy would it have been?  Did she add extra weight during her pregnancy?  O.k. I only know from my own experience...I had terrible sciatica due to the weight of my baby and also after due to the added weight I gained.  It was very bad with my first child as when they were given me an epidural, it hit a nerve or ?? and caused a "zinger" that immediately went right down my back to my hip and to my toe.  Not knowing anything about sciatica at the time...I later found out that this is what it was...I would continue to get what I called "phantom" pains for about 2 years and into my second pregnancy...the only relief I got was when I finally got the added weight off and also doing cat stretches...down on all 4's, tucking the pelvis under and arching the back...with my 3rd...the sciatica pain came back but not the "zinging" sensation...with my 4th...I had my tubes tied (way over due...haha!)  but can't figure out for the life of me, how that would affect the sciatica...they went in thru the belly button.  So, I would talk to your client more and try to find out more about her history.  I don't know if my story sheds some light but I would believe it to be all due to the added weight of a baby...if, indeed, she had delievered at the time...If that's not the case...hmmmm....just ask more questions.  But I highly recommend the "cat-stretch" for homework...it's great at targeting those areas in both the front and back...when done correctly.  Good luck!JoJo

Interesting...Did she have her tubes tied after she delivered her baby?  Did she have an epidural?  

I wondered the same thing. Suffered through sciatica with 2 pregnancies, though I did not have any table exams or epidural.  Having my hip 'dropped' at the chiro helped. Those stretches, also known as 'dromedary drops' also helped. You might want to teach and check for proper form before your client leaves.

Another thing that helped me was the side-lying relaxation position from Bradley...I'm having trouble finding an image to link. Any Doulas on the board who might have some input?

Another questionid she have her tubes tied after C-section delivery?That is a whole new ball of wax.  They pull out the intestines, get in there to do the ligation and then put all the parts back in.Not that that should cause a sciatica, but usually with the C-section you get the "block", so maybe it has something to do with that.

I have never heard of sciatica having to do with tying tubes. The scitic nerve comes out of your spine at the L4 L5 area then goes south from there. That's further back than falopian tubes. Sciatic pain is common amoung pregnancies mainly having to do with poor posture during the pregnancy. With the tendons pull on the PSIS and lower that causes stress and sciatic like pain. Also with all that weight to carry around the spine is bound to compress a little causing sciatica. I had a client that had sciatica from her pregnancy 16+ years before I had seen her. Sciatica doesn't go away and sometimes it doesn't even appear until after the pregnancy...usually it appears during. It's a matter of weight and posture I believe. I woudl question what the cause was of her sciatica. Talk to her doctor...with her permission of course. It may help to know exactly what you are woring on. It could be piriformis syndrome but most liekly sciatica due to her pregnancy....not tube tying.Well that's my 2 cents

That's technically incorrect.

We do deliver the Uterus but the bowel(large and small) is not involved.

The pregnancy itself or positioning( we have them in Lithotomy postion for a tubal) probably had to do with the sciatica.

I don't want to get in to a technical operations war about what some surgical teams do and other's don't, but I have a specific example of this occuring just three weeks ago, so it is a possibility.  The then delivering mother and family are medical background (doctors and a PT), and unless someone saw something incorrectly during the surgery (and I would be highly cautious to assume they did) then this does occur and would be something to think about.Could be why my friend is in such severe pain right now.
