Which Client Forms do you use??

Im getting ready to start my new practice tomorrow, and was just going to download client forms from this site:

But was struggling with which ones to use? THey all seem pretty good. I like the history form of course,but what about soap notes, client status, health info?, session notes AND soap notes???

just curious what you all find the most useful

I use an intake/history form, a consent form that includes all appt./financial policies plus privacy/mailing statement, and CARE notes (similar to SOAPs) - space for 4 sessions, 2 front 2 back. About once a year I have each client update their history forms, usually just adding in notes in a different color ink w/dates & initials.

Good point. Informed consent & understanding of policies is part of my intake form.

Another woman in my office has one form that serves as history, informed consent and policies. I think it's a smart move and I often wonder if my 4 pages (2 for history, a copy of my policies for them to take home and consent) is overkill. Granted, I do a lot of pain management/injury work so I like the extensive history. But I wonder if I should have a separate intake, like yours for those who come for relaxation/muscle tension relief from daily living issues? I tend to over think things and make more work for myself in the process. :roll:

One is for more detail, for those clients in pain due to injury, etc., one for strictly relaxation (still a full page of history, though),and one for seniors (leaves off questions like Are you pregnant? and has bigger print). The policy page is attached and their signature says they've read it, and they are welcome to pull that page to keep. I only have them sign once, but the paragraph preceding the signature covers it all.

It's hard not to end up with overkill, LOL.

Thank you for this! I never even thought of the larger print for my senior clients. Okay, it's good to know I'm not alone in multiple forms Thank you Rose!

One way I make the paperwork seem less is to mail it to new clients, so they can bring it to their first visit. It's only two pages total, but having it ahead of time lets them read the policies/consent thoroughly and really think about the history if they like, without feeling rushed.

This is a good idea. Do you find they take the time to fill it out before they come? I leave lots of time per appointment so they can take their time and be assured they will still get all their time on the table, but I do see them rushing through and signing the policies when I know they haven't read them all.

I use two primary forms. One is the intake form, 3 pages. The reason for 3 pages is that it includes all contact info, goals for today's session, a health history, a consent to each body area, a small laundry list of health conditions I should know if they have, and the consent statements.

The second item is a 1-page policy sheet, much like a doctor's office. It explains cancellation policy, payment methods, lateness, draping, etc.

We make both of these available in Microsoft format on our website, and we have all new clients access them and fill them out beforehand and just bring them with on their first visit.

I use other forms internally, such as SOAP notes and a "Client preferences" sheet that jogs my memory regarding special requests, uncomfortable positions, allergies, preferred scents or music, hot stone temp sensitivity, etc.

I made all of our forms myself, and they've changed a little over the years. They work well for us. I've never used any forms made by anybody else, though, so I can't speak for them
