The NHS have proposed a traditional discectomy at L5/S1 (accessed by laminotomy) and interspinous spacer (Wallis ligament) all done under general anaesthetic. This is open back surgery where the prolapsed section of the disc is cut away. A 3 to 4 day stay in hospital followed by recuperation for 4-6 weeks before returning to work is advised. They also say it may take 12-18 months to gain the full benefits of surgery.
The private surgeon proposes minimally invasive endescopic lumbar decompression and foraminoplasty, after spinal probing and discography at L4/L5 and L5/S1 have identified the pain producing level. This is keyhole surgery done in aware-state (IV sedation and local anaesthetic) and where the offending disc material is removed by laser. Hospital discharge is the day after surgery and normal activities and work are expected to resume in 6-8 weeks (after regular physio and gradual build up of activities).
After some deliberation (about 1 nanosecond) I have chosen the private route.
